CONSENT FORM
I, hereby volunteer as a Board Member as mentioned herein. Should my application be accepted after the necessary screening, I acknowledge and agree as follows:
1. As a board member I shall be in a fiduciary relationship to Rata Social Services, at all times act honestly and in good faith towards the organisation.
2. I shall exercise my powers and functions as a board member in the interest of the said organisation and shall not exceed my assigned powers and functions.
3. I shall do my utmost to avoid material conflict between my own interest and those of the organisation, not derive any benefit from my position, to which I am not entitled, whether directly or indirectly and shall not delay the notification of the Board of the nature and extent of any direct or indirect interest which I may have in any contract with the organization.
4. I am not disqualified from being appointed or acting as a board member in terms of the provisions of MOI
5. I undertake to acquaint myself with my statutory and regulatory duties as a Board member and to always act in accordance therewith.
6. I shall address all parties with respect, always maintain a professional relationship and act in a civil and courteous manner.
7. I confirm that I am able to meet the following requirements of the position:
a. Have access to electronic facilities / equipment and data for communication purposes and virtual meetings, such as email, WhatsApp and zoom.
b. Have transport to attend occasional meetings in person. (maximum four per year)
c. Prepared to be actively involved in the activities of the Board.
